I agree that the children and I will follow all posted safety signs, rules, and verbal instructions, and that I will report any hazards I observe. I understand there are inherent risks of injury at The Monkey’s Treehouse. By using its facilities, I accept these risks for myself and the listed children. I will not assert a claim if injury occurs, and I will indemnify and hold harmless The Monkey’s Treehouse (MJ Treehouse Ventures LLC) and its owners if another person brings a claim related to these children. By paying and selecting this option, I confirm I am an adult parent/guardian/caregiver of the listed children, this agreement is legally binding, and I consent to the use of photos or videos taken for marketing.
